This form is printed by the Chinese quarantine authority and must be completed in English. The applicant should complete each item of the examination in strict accordance with the requirements of the examination form. Incomplete records or those without a photograph of the applicants, without the signature of a doctor, or the official stamp of the hospital are invalid, and the examination results are valid for six months.
3. Applicants who have given up their original Chinese citizenship and joined foreign citizenship and both or one of whose parents have Chinese citizenship should submit

Transcripts in languages other than Chinese or English should be translated into Chinese or English and be certified by notarization.

Graduation certificate in languages other than Chinese or English should be translated into Chinese or English and be certified by notarization.
The graduates of the current year can provide pre-graduation certificate, after being admitted, the original must be submitted.
*10. Two reference letters from professors
Which should include the aim and requirements of studying in China, an evaluation of the applicant’s overall ability and potential. The reference letters should be written in Chinese or English, with the professor’s signature and contact information.
*11. Proposal of the study in China
In Chinese or English.
Tips: For applicants who are in China: If you are an employee, you still need to provide employment separation certificate and your current residence permit. If you are a student, you need to provide transfer certificate, leaving certificate, certificate of attendance rate issued by previous school and your current residence permit.
